How do you ask for the care allowance in Czechia?
You file the care allowance at the labour office for the place of permanent residence. The form page asks for identity, or a birth certificate for a child under 15, and the notice of who will provide the help. A hospital note is optional, and an income note is named only when you also ask for an increase.
