How do you ask for the birth grant in Czechia?

You file the birth grant at the labour office. The form page asks for identity for each person named, or a birth certificate for a child under 15, and quarterly income or a declaration when there is no income. A court decision, a maintenance note, and a study or health note are named only when that case applies.
