What papers do you gather for a child's passport in the United States?
The State Department says a child under 16 applies in person with both parents or guardians, on Form DS-11, with a paper proof of citizenship and a photo ID for each parent. You cannot renew that passport. The photo, the photocopies, and the consent form if one parent cannot attend are on the official page.
