The federal Medicaid directory lists the Medicaid agency for New York as New York State Department of Health, at www.health.ny.gov/health_care/medicaid. The enrollment page it prints is www.health.ny.gov/health_care/medicaid/how_do_i_apply.htm. The phone line says to call 855-355-5777 if you enrolled through NY State of Health.
