advance healthcare directive 是一份书面说明：如果以后你不能自己说，你想要或不想要哪些医疗或手术。制作时你必须年满 18 岁，并且还能做决定。制作、修改和取消都免费。必须写成文字，并在 2 名见证人面前签字。你可以指定一名年满 18 岁的 designated healthcare representative，说明你的意愿，并同意或拒绝治疗。你不必指定。如果指定了，对方也要签字。对指名治疗的拒绝，如果符合当时情况，可以有法律约束力。要求某种治疗必须被考虑，但没有约束力。拒绝治疗不是要求别人结束你的生命。enduring power of attorney 不包括这些治疗决定。Decision Support Service 发布了模板。给 GP 一份副本。请在官方页面上确认。
