advance decision 让你事先拒绝某一种治疗，以免以后自己说不出来。NHS 也把它叫做 advance decision to refuse treatment，或 living will。这不是房子或钱的遗嘱。也不是 lasting power of attorney。年满 18 岁并且还能决定时，由你自己写。写明每一种治疗，以及在什么情况下拒绝。如果拒绝的是可能维持生命的治疗，必须写成文字，由你签字，并由一名见证人签字。必须写明即使生命有危险也适用。告诉家人和医生它放在哪里。可以要求放一份进病历。拒绝治疗不是要求别人结束你的生命。请在官方页面上确认。
